Waiver
I wish to volunteer with xHope, Inc. As a volunteer, I hereby release, indemnify, and hold harmless xHope, Inc., its directors and board members, staff, successors, volunteers, legal representatives, organizers, sponsors, and supervisors of its activities from any and all claims, causes of actions and liability arising from or in any way connected with my volunteer participation with xHope, Inc.
I understand that xHope, Inc. may request to use photos and/or videos of its volunteers, program participants and organization partners, taken during program activities and events to use in print, multimedia or web form. Photos/videos will only be used only for purposes related to xHope, Inc.'s programs and its affiliated partners. I hereby grant permission to xHope, Inc., its partner organizations and affiliates, to use my photo/video and likeness and/or any video footage or written/verbal statements from me, in its print publications, social media postings, and any other means used to publicize its programs. This consent includes, but is not limited to permission to interview, film, photograph, tape or otherwise make a video reproduction of me and/or record my voice. I understand that xHope, Inc. is a non-profit organization, and that photos published will not be used for any commercial purposes.
For the safety and well-being of all participants, and to provide an empowering environment for the youth and families we serve, volunteers are expected to maintain a substance-free presence while representing xHope. This means:
No smoking or vaping
No use or possession of illegal drugs
No consumption of alcohol
Volunteers who violate this policy may be asked to leave the premises and may be prohibited from future volunteer opportunities.
By signing below, I acknowledge and agree to abide by this policy.
